Preventing disease, promoting healthy choices, and recognizing personal risks factors for developing disease are critical to maintaining health and longevity.
It is common for most of us to believe we are healthy and so we do not need to go to the doctor; the reality is we all benefit from regular health maintenance.
I hope to create a sense of meaning and significance to preventative care and benefit of health screening.
I offer full health screenings, exams, recommended population based care for men and women and ages 15 years and up.

The goal is to prevent chronic illnesses from developing, but when that doesn't happen it is important to reduce the harm and progression of the disease on other crucial systems and organs.
Some examples: Hypertension/high blood pressure, heart/lung disease and diabetes
I offer comprehensive chronic care management of these diseases, working with specialists and communicating with outside providers to better serve you and prevent complications/hospitalizations.

Ways We Can Connect

Home Visits
Home visits are what they seem to be. I travel to your home, a place you reside, or a place you feel most comfortable that is outside the traditional clinic environment. These are to establish care, complex chronic care and visits that require more in-depth care coordination, such as working with a facility or caregiver.
-
Traditional Clinic Based Visits
These are traditional clinic-based visits.
I will be hosting visits on Fridays & Saturdays with extended hours to meet the needs of the community.
-
Telehealth Visits
These visits are available for established patients only. These visits provide an easy and flexible opportunity for follow-ups, acute concerns that do not require a physical exam and in-between chronic care visits.
